There's a concept in cell biology called autophagy - from the Greek auto (self) and phagein (to eat). It's the process by which your cells identify their own damaged components - broken mitochondria, misfolded proteins, accumulated debris - and break them down for recycling. It's cellular housekeeping. And according to Dr Mindy Pelz, one of the most effective ways to activate it is something people have been doing for as long as our species has existed: not eating for a while.

The Fasting Timeline

Pelz describes fasting as a progression of metabolic states, each unlocking different repair processes. The timeline isn't exact - individual variation matters - but the broad sequence is consistent across the research:

0-12 hours: Your body is processing its last meal. Glucose and insulin levels are elevated. You're in "fed state" - burning glucose, storing fat, and running normal digestive processes.

12-14 hours: Glucose stores in the liver (glycogen) are depleted. Insulin drops. The body begins shifting to fat oxidation - breaking down stored fat into fatty acids and ketones for fuel. This is the metabolic switch.

17 hours: Autophagy ramps up significantly. With insulin low and energy demands shifting, cells start prioritising repair over growth. Damaged proteins and cellular components are tagged for recycling. This is the cellular maintenance window that eating every few hours prevents.

24 hours: Intestinal stem cells begin a reboot process. Research in mice published in Cell Stem Cell showed that a 24-hour fast doubled the regenerative capacity of intestinal stem cells. Pelz describes this as a gut reset - the lining of your intestines turns over rapidly, and fasting appears to accelerate the quality of that regeneration.

36-48 hours: Growth hormone increases substantially (some studies report several-fold increases). The dopamine system begins to recalibrate, which Pelz connects to breaking cycles of compulsive eating. Extended fasts at this duration are less practical for most people and should be undertaken with awareness and, ideally, guidance.

Why Constant Snacking Works Against You

The modern advice to eat small meals every 2-3 hours was well-intentioned - it was designed to keep blood sugar stable and metabolism active. But the unintended consequence is that insulin stays elevated for most of the waking day, which keeps autophagy suppressed. Your cells never get the low-insulin signal they need to shift into repair mode.

This doesn't mean snacking is inherently bad. For some people - particularly athletes in heavy training, pregnant women, or those managing hypoglycaemia - frequent eating is medically appropriate. But for the average office worker eating breakfast at 7am, a mid-morning snack at 10am, lunch at 1pm, an afternoon snack at 3pm, and dinner at 7pm, insulin is elevated for roughly 14 hours straight. That leaves a very narrow window for cellular maintenance - most of which overlaps with sleep, when repair processes are already competing for resources.

The Overnight Fast You're Already Doing

The word "breakfast" literally means "break fast." Every night, between your last bite of food and your first bite in the morning, you're already fasting. The average person gets about 10-11 hours of overnight fast. The question is whether extending that by 3-6 hours has meaningful health benefits - and the evidence increasingly says yes.

A 2019 review in the New England Journal of Medicine examined decades of research on intermittent fasting and concluded that fasting periods of 16-24 hours are associated with improved insulin sensitivity, reduced inflammation, enhanced cellular repair, and better cardiovascular risk markers. The improvements were observed even without changes in total caloric intake - meaning the timing of eating mattered independently of the amount.

Getting Started

The simplest entry point: close the kitchen after dinner and don't eat again until lunch. If dinner finishes at 7pm and lunch starts at noon, that's a 17-hour fast - right at the autophagy threshold. Black coffee, plain tea, and water are fine during the fasting window. If 17 hours feels too aggressive, start at 14 and extend gradually over a few weeks.

The goal isn't to become someone who never eats. It's to create regular windows where your body can do the maintenance work that constant food intake prevents. Think of it like sleep for your cells - not something you do all the time, but something you can't skip without consequences.
