The Vitamin Deficiency That Rivals Smoking

Dr Rhonda Patrick is a biomedical scientist with a PhD in nutritional science, and the founder of FoundMyFitness. Her research focuses on how micronutrient deficiencies accelerate aging and chronic disease. When she appeared on The Diary of a CEO, she led with a claim that's hard to walk past: having a low omega-3 index is roughly as predictive of early death as smoking a pack of cigarettes a day.

That sounds like the kind of exaggeration designed to sell fish oil capsules. But the research she cites is from large, peer-reviewed epidemiological studies - and the comparison, while provocative, is rooted in data on telomere shortening, inflammatory markers, and cardiovascular mortality.

Omega-3: The Numbers

The omega-3 index measures the percentage of EPA and DHA (two specific long-chain fatty acids) in your red blood cell membranes. An optimal index is above 8%. Below 4% is considered deficient - and in Western populations, average levels sit somewhere between 3% and 5%.

Low omega-3 levels are associated with shorter telomeres (the protective caps on chromosomes that shorten with age), increased systemic inflammation, higher rates of cardiovascular disease, and - in several longitudinal studies - higher all-cause mortality. Patrick recommends 1-2 grams of combined EPA and DHA daily from fish oil or algae-based supplements, which is broadly in line with guidance from the American Heart Association.

The food-first version: two servings of oily fish per week - salmon, mackerel, sardines, herring - typically delivers enough. If fish isn't a regular part of your diet, supplementation becomes more relevant.

A meta-analysis of 13 randomised controlled trials involving over 127,000 participants found that marine omega-3 supplementation was associated with significantly lower risk of heart attack and coronary heart disease death, with a clear dose-response relationship. [Hu et al., JAHA, 2019]

Vitamin D: The 70% Problem

Patrick cites data suggesting that roughly 70% of Americans - and a similar proportion of people in Northern Europe, including Ireland - have insufficient vitamin D levels. In Ireland specifically, the issue is worse during winter months when UVB radiation from sunlight is too weak to stimulate vitamin D synthesis in the skin from roughly October to March.

The consequences of long-term deficiency are not trivial. Patrick references research showing that vitamin D deficiency is associated with an 80% increase in dementia risk, while supplementation with 2,000-4,000 IU daily can reduce that risk by approximately 40%. Vitamin D is involved in immune function, bone health, and inflammatory regulation - and at the population level, most people in Northern Europe are not getting enough of it during half the year.

The FSAI recommends that all adults in Ireland consider taking a vitamin D supplement between October and March.

Magnesium: The Quiet Deficiency

Magnesium is involved in over 300 enzymatic reactions in the body, including DNA repair, muscle function, and nervous system regulation. Despite this, an estimated half of the population in Western countries doesn't meet the recommended daily intake. It's one of those nutrients where you're unlikely to feel acutely "deficient" - the symptoms are vague: poor sleep, muscle cramps, low mood, difficulty concentrating. They overlap with a dozen other things, which is why the deficiency goes unrecognised.

Patrick highlights observational data showing that high magnesium intake is correlated with 40% lower all-cause mortality and 50% lower cancer mortality. These are associational numbers, not proof of causation - people who eat magnesium-rich diets (leafy greens, nuts, seeds, dark chocolate, whole grains) also tend to have other healthy habits. But the consistency of the signal across multiple large studies is notable.

What About Creatine?

Perhaps the most unexpected recommendation from Patrick: creatine supplementation at 5-10 grams daily, not for muscle building, but for cognitive performance. Creatine serves as a rapid energy buffer for cells with high metabolic demands - which includes brain cells. Small but consistent studies have shown improvements in short-term memory and processing speed, particularly under conditions of sleep deprivation or mental fatigue.

Patrick also addresses the persistent myth that creatine causes hair loss - a claim based on a single small study that has not been replicated. The current evidence suggests creatine is one of the most well-studied and safe supplements available, with benefits that extend beyond the gym.

The 70% Rule

The thread connecting all of Patrick's work is a single statistic: lifestyle choices account for roughly 70% of how we age. Genetics play a role, but a smaller one than most people assume. The identical twin studies she references consistently show that twins diverge significantly in health outcomes based on diet, exercise, sleep, and supplementation - not inherited predisposition.

That's simultaneously the good news and the hard news. It means the leverage is yours. It also means the responsibility is yours.
