Protein, Carbs, Fat: What the Evidence Actually Says About Each
Decades of conflicting dietary advice have left most people confused about the basics. Here's what nutritional science has settled on - and where genuine debate remains.
Nutritional science has a reputation for contradicting itself - eggs are bad, then fine, then beneficial; fat is the enemy, then carbs are, then neither. Some of this reflects the genuine difficulty of conducting long-term dietary research in humans. But much of it reflects media amplification of preliminary findings, industry funding of selective research, and the natural process of scientific revision being mistaken for randomness. The settled science on macronutrients is actually more stable than the headlines suggest.
Carbohydrates: Quality Matters More Than Quantity
The case against carbohydrates as a category was never particularly strong, and it hasn't strengthened over time. EFSA's Dietary Reference Values recommend that 45–60% of total daily energy intake come from carbohydrates for adults. The more important distinction, well-supported by evidence, is between refined and unrefined sources.
Refined carbohydrates - white bread, white rice, added sugars - are metabolised rapidly, producing faster blood glucose rises and contributing fewer micronutrients per calorie. Whole grains, legumes, vegetables, and fruit provide carbohydrates packaged with fibre, vitamins, and minerals that slow digestion and improve the metabolic response. The evidence for the health benefits of high fibre intake is among the most consistent in nutritional epidemiology: a 2019 Lancet meta-analysis covering 135 million person-years of data found that higher dietary fibre intake was associated with reduced risk of cardiovascular disease, stroke, Type 2 diabetes, and colorectal cancer.
Fats: The Saturated Fat Question
The demonisation of all dietary fat in the 1980s and 1990s was a significant error, and most nutritional bodies have quietly walked it back. EFSA's current guidance makes the distinction clearly: unsaturated fats - monounsaturated fats in olive oil, polyunsaturated omega-3 and omega-6 fatty acids in oily fish, nuts, and seeds - are associated with positive cardiovascular outcomes. Saturated fats - from butter, fatty meat, full-fat dairy, and tropical oils - raise LDL cholesterol when consumed in excess, which is associated with elevated cardiovascular risk.
Trans fats - found in partially hydrogenated oils and some processed foods - have the clearest negative evidence profile and have been progressively regulated out of European food products.
The practical conclusion: replacing saturated fat with unsaturated fat improves cardiovascular risk markers. Replacing saturated fat with refined carbohydrates doesn't show the same benefit. Total fat intake in a healthy diet is less important than fat quality.
Protein: How Much Do You Actually Need?
As covered in the protein-per-euro article, EFSA's Dietary Reference Value for protein is 0.83g per kilogram of bodyweight per day for adults - based on nitrogen balance studies. For people doing regular resistance exercise, the evidence supports higher intakes of 1.2–1.6g/kg for muscle maintenance and growth, per a 2017 meta-analysis in the British Journal of Sports Medicine.
The type of protein matters somewhat - animal proteins tend to have higher bioavailability and complete amino acid profiles. Plant proteins can fully meet requirements when varied and combined, but require more attention to adequacy, particularly for lysine (found in legumes, not grains) and methionine (found in grains, not legumes) - which is why combining the two is nutritionally significant.
