Why Dairy Keeps Giving You Breakouts
The link between dairy and acne is one of the most studied and most ignored connections in nutritional dermatology. Multiple large studies confirm it. Here is exactly what happens when milk meets your skin.
Dairy is one of those foods that people do not want to suspect. It is deeply embedded in how most of us eat - milk in coffee, cheese on everything, yoghurt as a healthy snack. But the evidence linking dairy to acne has been building for over two decades, and at this point, it is difficult to dismiss.
This does not mean dairy causes acne in everyone. It means that if you have persistent acne that responds poorly to topical treatments, dairy is one of the first things worth investigating.
The Nurses' Health Study: Where It Started
The most influential evidence came from Harvard. Researchers analysed dietary data from over 47,000 women in the Nurses' Health Study II and found a clear positive association between dairy intake during high school and subsequent acne. Skim milk showed a stronger association than whole milk, which was unexpected and pointed away from a simple fat-related mechanism.
A follow-up study looked at over 6,000 girls aged 9-15 and found the same pattern. Higher milk intake, particularly skim milk, was associated with more acne. Total dairy consumption was positively associated with acne prevalence.
In a cohort of over 47,000 women, those who consumed two or more servings of skim milk daily during adolescence had a 44% higher risk of physician-diagnosed severe acne compared to those consuming one serving per week or less. [J Am Acad Dermatol, 2005]
Why Skim Milk Is Worse Than Whole Milk
This finding confused people at first. If dairy fat were the problem, whole milk should be worse. But the issue is not the fat. It is the hormones.
Milk naturally contains over 60 different hormones and growth factors, including IGF-1, insulin, and various androgens. When you remove the fat to make skim milk, the concentration of these bioactive compounds relative to the liquid volume actually increases. Furthermore, some of the hormones are water-soluble, not fat-soluble, meaning they remain in skim milk at full concentration.
IGF-1 is the primary suspect. It stimulates the sebaceous glands to produce more oil, accelerates skin cell proliferation (creating more material to clog pores), and amplifies androgen receptor sensitivity. Drinking milk essentially supplements your body with additional growth signalling that your skin did not need.
The Whey Protein Problem
If you work out and use whey protein supplements, this section is particularly relevant. Whey is derived from milk and is rich in insulinogenic amino acids - leucine, isoleucine, and valine. These amino acids are potent stimulators of insulin and mTORC1, the same cellular pathway that drives sebum production and acne formation.
Multiple case reports and small studies have linked whey protein supplementation to new-onset acne or worsening of existing acne, particularly along the jawline and back. If you started breaking out after adding a protein shake to your routine, the whey may be contributing more than the workout is helping.
What About Fermented Dairy?
Yoghurt and kefir present an interesting exception. The fermentation process partially breaks down the proteins that cause immune reactions, and the beneficial bacteria may offset some of the inflammatory effects. Several studies have found weaker or no associations between fermented dairy and acne, compared to the strong associations seen with milk.
This does not make fermented dairy free from suspicion for everyone. But if you are trying to reduce dairy and struggling with the idea of giving up everything, yoghurt and kefir are the last items to cut, not the first.
What to Do About It
The simplest test is a 30-day dairy elimination. Remove all dairy - milk, cheese, butter, cream, yoghurt, whey protein - for one full month. Document your skin weekly with photos taken in the same lighting. After 30 days, reintroduce dairy and observe for 3-5 days.
If your skin improves during the elimination and worsens on reintroduction, you have your answer. You can then experiment with which dairy products are most problematic (liquid milk is usually the worst offender) and which you might tolerate in small amounts.
A 2018 meta-analysis of 14 studies involving over 78,000 participants confirmed that dairy consumption, particularly milk and skim milk, is significantly associated with increased acne risk in both adolescents and young adults. [Nutrients, 2018]
